What do i do if I Received a bill from provider?
1. If the bill doesn’t have procedure codes, call the provider and request an invoice that includes codes.
2. Download Explanation of Benefits (EOB) for the same dates of service from Curative. Without these documents, we cannot move forward.
3. Always check that the reason for non payment is “additional insurance”. If so, call Curative and tell them you do not have any other insurance and to reprocess the claim.
By law we cannot access your medical information.
Who is our dental & Vision insurance provider?
Aetna provides our dental and vision coverage.
